Vizzini a9aabdaaf1 Patch by skywing:
- wrote RtlRaiseException user mode implementation
  - wrote KiRaiseUserExceptionDispatcher/KeRaiseUserException to raise user exceptions from kernel mode
  - implemented raise STATUS_INVALID_HANDLE-if-debugged in NtClose
