[NTOS:PNP] Implement PlugPlayControlStartDevice control class
This control class is triggered when a driver is being installed for a
non-critical device. The driver info should already be in the registry
so we just need to push the device through the state graph

Meanwhile, combine the code for similar control classes into
PiControlSyncDeviceAction routine

[NTOS:SE] Do not use a global lock for tokens (#3445)
In Windows Server 2003 the lock is initialised on a per-token basis, that is, the lock resource is created in SepDuplicateToken() and SepCreateToken() functions. This ensures that the lock initialisation is done locally for the specific token thus avoiding the need of a global lock.

Jérôme Gardou d8cdb89fb0 [NTOSKRNL] Overhaul Cc and Mm relationship
Previously, when creating a file section, Mm requested Cc to cache the file, then Cc would request pages from Mm, then Mm would request them back to serve its file-mapping role
Now, Mm does it all by itself. If file cahcing is requested by the FS driver, then Cc creates a file mapping and uses that to serve its purpose.

This is a rewrite of Cc

ProxyData and AuditData are pointers to an arbitrary data, which instead, they should point to their respective data structures. This serves as preparation for SepFreeProxyData and SepCopyProxyData functions implementations in the future (regarding the proxy data stuff specifically).

During the boot process, it makes possible to initalize the driver's
devices right after the driver is loaded. Moreover, this way one can be
sure that all critical devices are initialized before the
IopMarkBootPartition call (because we explicitly call the driver's
AddDevice routine now, after each driver is loaded)

[NTOS:PNP] Implement NT5.2-like DEVICE_NODE state management
- Use DeviceNode->State field and its values, instead of
  DeviceNode->Flags for tracking current node state
- Change DNF_* flags to the ones compatible with Windows XP+
- Simplify state changes for device nodes and encapsulate all the logic
  inside the PiDevNodeStateMachine routine. This makes the ground for
  future improvements in the device removal sequence and
  resource management
- Now values inside DeviceNode->State and ->Flags are compatible with
  the windbg !devnode macro and can be tracked using it
- BUGFIX: fixed cases where IRP_MN_START_DEVICE or
  IRP_MN_QUERY_DEVICE_RELATIONS may be sent to a device after a
  IRP_MN_REMOVE_DEVICE

- Move the driver's name obtaining logic into the IopGetDriverNames
  function
- Create a new PiCallDriverAddDevice instead of PipCallDriverAddDevice
  and move it to pnpmgr/devaction.c file. Move around all its internal
  helpers too
- Support a proper Windows-compatible driver loading order for a PDO
  (lower filters, main service, upper filters, etc.)
- Set a correct Problem for the DeviceNode, in case of an error during
  driver loading
- Check the Start Type for all drivers before loading
- Do not try to load drivers during the early boot stage when there is
  no disk subsystem initialized

  supposed to be done before the function call

[NTOS:IO] Refactoring of the driver initialization code
- Remove IopCreateDriver and put its code into IoCreateDriver and
  IopInitializeDriverModule. It's hard to extract a meaningful common
  part from it
- Refactor IopInitializeDriverModule. Extend and put the DriverName
  generation logic into it. Now this function frees the ModuleObject in
  case of failure and returns STATUS_FAILED_DRIVER_ENTRY in case of
  DriverInit failure (will be used later)

- BUGFIX: do not call IoGetRelatedTargetDevice while guarded mutex is acquired
  (the function issues an APC, but they are disabled inside a critical section)
- BUGFIX: only the beginning of a structure for GUID_PNP_CUSTOM_NOTIFICATION was copied and queued.
  Just pass it as-is to a subscriber, without copying
- Don't convert event GUID to string, store and compare GUID struct itself
- Split IopNotifyPlugPlayNotification into 3 functions for each type of notification
  (less stack usage and for future changes)
- Move initialization code for notifications into a separate routine
- Use separate lists and locks for every type of notification
- Put "TargetDeviceChange" notifications into their place inside DEVICE_NODE

Thomas Faber 4f49a9c792
[NTOS:KE] Save ExceptionList in the assembly version of KiEnterTrap. CORE-15723
If SEH is used in a C trap handler, the exception frame will be
registered before the call to KiEnterTrap, which means we save
the wrong trap handler. We'll therefore also restore this wrong
frame for the excepting code, resulting in a stale SEH chain.

We avoid this problem by saving the handler in the assembly
trap entry code instead of from C. While SEH in a C trap handler
should now theoretically be safe, we still forbid it through
asserts in the C KiEnterTrap variants to make any potential
future problems more obvious. Should this functionality be
needed at some point and deemed safe, these asserts can then be
removed.

[NTOS:KD/KD64/KDBG] Share some code between our legacy KD/KDBG and KD64.
Our legacy KD module is slowly being phased out for the more recent KD64
Kernel Debugger that supports WinDbg, but at the same time we must retain
support for GCC debugging and the KDBG interface.

For the time being few #ifdef _WINKD_ have been introduced in KD64 so that
some of its code/data does not completely get shared yet with the legacy KD,
until the latter becomes phased out.

KD Modifications:
=================
- Remove the implementation of NtQueryDebugFilterState() /
  NtSetDebugFilterState() that now comes entirely from KD64.

- Remove KD variables that are now shared with KD64.

- Share common code with KD64: KdpMoveMemory(), KdpZeroMemory(),
  KdpCopyMemoryChunks(), KdpPrint(), KdpPrompt().

- KDBG: Remove the duplicated KdpCopyMemoryChunks() function.

- In KdpServiceDispatcher() and KdpEnterDebuggerException(), call the
  KdpPrint() worker function that correctly probes and captures its arguments.

- Temporarily stub out KdEnterDebugger() and KdExitDebugger() that is used
  by the shared code, until KD is removed and only the KD64 version of these
  functions remain.

- Re-implement the KD/KDBG KdpPrompt() function using a custom KdpPromptString()
  helper compatible with KD64, that is called by the KD64 implementation of
  KdpPrompt(). This KdpPromptString() helper now issues the prompt on all
  the KD loggers: e.g. if you use both at the same time COM-port and SCREEN
  debugging, the prompt will appear on both. Before that the prompt was always
  being displayed on COM port even if e.g. a SCREEN-only debug session was used...

- ppc_irq.c: Fix the prototype of KdpServiceDispatcher().

KD64 Fixes:
===========
- Initialize the MaximumLength member of the counted STRING variables
  before using them elsewhere.

- Get rid of alloca() within SEH block in KdpPrint() (addendum to 7b95fcf9).

- Add the ROS-specific handy dump commands in KdSystemDebugControl().

[NTOSKRNL] Rewrite IoAssignDriveLetters to make NT5 compliant
The major change with this rewrite is the support for the mount
manager. Fstub will now assume that most of the devices are PnP
and that they are already registered to the mount manager.
It will thus ask the mount manager to assign the drive letter.
Fstub will keep assigning drive letters non mission critical devices
such as CDs, floppies and other removable devices.

See MountMgr:QueryPoints API test that will now return mount points :-).

- Introduce the MmMakeKernelResourceSectionWritable() helper for
  making the kernel resource memory section writable, and use it
  in KeGetBugMessageText(). Indeed, this latter function patches
  in place the bugcheck resource message to trim any trailing
  newlines before displaying the message on screen.

  that mentions it too.

  This fixes bugcheck text display (e.g. the MANUALLY_INITIATED_CRASH one)

shared cache map. Right now, we only perform global
operation.

  immutable lists of user & groups and privileges reside, and the
  "dynamic information area" (allocated separately in paged pool),
  where mutable data such as the token's default DACL is stored.
  Perform the necessary adaptations in SepDeleteToken() and in
  NtSetInformationToken().

- Actually dereference the token's logon session, when needed, in the
  'TokenSessionReference' case in NtSetInformationToken().

- Overhaul SepFindPrimaryGroupAndDefaultOwner() so that it returns
  the indices of candidate primary group and default owner within the
  token's user & groups array. This allows for fixing the 'TokenOwner'
  and 'TokenPrimaryGroup' cases of NtSetInformationToken(), since the
  owner or primary group being set *MUST* already exist in the token's
  user & groups array (as a by-product, memory corruptions that existed

[NTOS:MM] Do not map two pages into hyperspace in MiCopyFromUserPage. CORE-14548
Doing this is not only wrong because it acquires the same spinlock twice,
it also completely breaks the TLB flushing logic in MiMapPageInHyperSpace.
If the PTE with Offset 1 is still valid when a wrap-around to 0 happens,
the TLB flush on wrap-around will not clear the entry for this previous page.
After another loop around all hyperspace pages, page 1 is re-used but its
TLB entry has not been flushed, which may result into incorrect translation.

[NTOSKRNL] Misc fixes to VACB reference counting
This fixes various bugs linked to VACB counting:
- VACB not released when it should be
- Reference count expectations not being accurate

For the record, VACB should always have at least a reference
count of 1, unless you are to free it and removed it from
any linked list.

This commit also adds a bunch of asserts that should
help triggering invalid reference counting.

It should also fix numerous ASSERT currently triggered and
may help fixing random behaviours in Cc.

The reserve IRP is an IRP which is allocated on system boot and kept during
the whole system life. Its purpose is to allow page reads in case of
low-memory situations where the system doesn't have enough memory left
to allocate an IRP to read from the page file (would be catastrophic situation).

[NTOSKRNL] Bring an initial (and not perfect ;-)) implementation of read ahead to our Cc!
This halfplements CcScheduleReadAhead() which is responsible for finding the next reads
to perform given last read and previous reads. I made it very basic for now, at least
to test the whole process.
This also introduces the CcExpressWorkQueue in the lazy writer which is responsible
for dealing with read ahead items and which is dealt with before the regular queue.
In CcCopyData(), if read was fine, schedule read ahead so that it can happen in background
without the FSD to notice it! Also, update the read history so that scheduling as a
bit of data.
Implement (à la "old Cc" ;-)) CcPerformReadAhead() which is responsible for performing
the read. It's only to be called by the worker thread.

Side note on the modifications done in CcRosReleaseFileCache(). Private cache map
is tied to a handle. If it goes away, private cache map gets deleted. Read ahead
can run after the handle was closed (and thus, private cache map deleted), so
it is mandatory to always lock the master lock before accessing the structure in
read ahead or before deleting it in CcRosReleaseFileCache(). Otherwise, you'll
just break everything. You've been warned!

This commit also partly reverts f8b5d27.

[NTOSKRNL] Properly implement lazy writer activity watching.
We get rid of the old iLazyWriterNotify event in favor of work items
that contain an event that lazy writer will set once its done.
To implement this, we rely on the newly introduced CcPostTickWorkQueue work queue
that will contain work items that are to be queued once lazy writer is done.

Move the CcWaitForCurrentLazyWriterActivity() implementation to the
lazy writer file, and reimplemented it using the new support mechanisms
